At your convenience before your next follow-up, complete the following steps in order to be prepared for the discussion.
- Send your practitioner a picture of the front and back of your General Intake Form.
- Complete this Medication List Form.
- Watch the following videos:
- Use the FAQ library of videos as a resource for continued learning.
Sometime on the day of your follow-up:
- Send your practitioner a picture of the inside of your chart.
- Complete the Observational Routine Form so that your practitioner can assess how thoroughly you are performing the observational routine.
