• Complete the Personal Evaluations Form (if it wasn’t completed verbally).

At your convenience before your next follow-up, complete the following steps in order to be prepared for the discussion.

• Watch the following videos:
  • Special Circumstances
• Use the FAQ library of videos as a resource for continued learning.

Sometime on the day of your follow-up:

• Send your practitioner a picture of the inside of your chart.
• Complete the Observational Routine Form so that your practitioner can assess how thoroughly you are performing the observational routine.
